【Rhinoplasty Revision Case】The patient had previously undergone one ear cartilage plus silicone rhinoplasty and one half-rib plus ePTFE rhinoplasty. This is the third revision. Frontally, the nose is not refined; the nasal tip is large, the alar rim is retracted, and the seagull line is poor in shape.
On profile, the nasofrontal angle transition is not distinct, the nasal tip defining points lack detail, the nasal tip is drooping, the lower lobule is underfilled, the ala is slightly drooping, and the midface is depressed.
On basal view, the alae are flared outward, and the alae and nostrils are asymmetric.
The half-rib plan: frontally reduce the nasal tip and alae, create a glabellar triangle transition, provide subtle dorsal undulation, remove hyperplastic tissue at the nasal tip, increase lower lobule fullness, reshape the supratip break, tip defining point, and lower lobule, perform alar rim pull-down, and adjust the seagull line

【Long Alar Shortening Case】
An overly long ala is a detail that is easily overlooked.
